Preservation Awards Nomination Form
For each nomination, please include a full set of the listed items here:
A completed nomination form, providing information on the nominee, nominator, and award category.
Descriptive narrative explaining how the nominee meets the award category criteria. The descriptive narrative should not exceed two pages.
Supporting materials, such as photographs, flyers, articles, or other materials documenting the project or person's work. Supporting materials will not be returned.
Digital photograph of the nominee to be used during the award ceremony, if selected.
If you are nominating a publication, nominations must include a copy of the work.
Optional: You may submit up to three letters of support for the nomination.
Nominations may be submitted by completing the following digital form and emailing the required attachments to josborne@bluegrasstrust.org.
Nominations will also be accepted by mail or in-person drop off.

All award recipients will be notified by email by the end of April. Award recipients should plan to attend the Blue Grass Trust Preservation Awards on Sunday, May 19, 2024, at the Dr. Thomas Hunt Morgan House in Lexington, KY. 2024 Preservation Awards Nomination Form
Please make sure to submit / upload the following supporting documentation:
1. Nomination form.
2. Descriptive narrative not to exceed more than two pages.
3. Supporting materials specific to the award category.
4. Digital photograph of the nominee. Please send digital images as a .JPG or .PNG with a minimum resolution of 300dpi.
5. Digital photograph(s) of the nominated project, program, or publication demonstrating the contributions of the individual or group.
6. Optional: Up to three letters of support.
